Credentialing Requirements
We require practitioners to meet specific credentialing and business requirements:

Education
Have graduated from an accredited college or formal training program.

License
Hold a valid unrestricted state license for licensed specialties.

Credentialing Criteria
Meet the credentialing criteria for WholeHealth Living Networks
for their specialty as outlined on the Practitioner application.

Liability Insurance
Agree to maintain professional liability insurance when applicable
with a minimum of $200,000 per occurrence and $500,000 aggregate.
Limits of 1M/3M are required for all MD's and DO's and ND/DC acupuncture practitioners.
Practitioners who participate with certain Health Plans may be required to have higher limits.

and credentialing application attestation statements.
